Your 1st Visit

Patient Forms

When you first enter our office, you will be greeted by our friendly office staff.  You will be given all the necessary patient forms to complete.  These forms provide us with information regarding your health history and information of your current conditions.  For convienience we have provided these forms on the website for you to fill out prior to your visit.


Next, you will have a consultation with Dr. Jason Judd.  This will be an opportunity for you to discuss your health-related problems and concerns.  By learning about you, your condition, and what you expect from treatment, Dr. Judd will be able to determine how chiropractic care can meet your goals.


After your consultation, Dr. Judd will perform a complete and thorough examination to determine the root cause for your condition.  By this examination he will be able to formulate a treatment plan that will be customized to you and your individualized needs. 

X-Ray Studies

Dr. Judd does not require all new patients to recieve an x-ray exam.  Therefore he does not have an X-ray machine in his office. Depending on your condition he may feel the need to refer you to a local walk-in X-ray clinic.  He will receive the results of your X-ray within 24 hours of the completed X-ray exam. 

Report of Findings

Once Dr. Judd has collected the information from his examinations and any X-rays that may have been completed, he will be able to give you a detailed report of his findings and answer any questions and concerns you may have including:

  1. What is my problem?
  2. How can Dr. Judd help me?
  3. How often do I need to come in?
  4. What will my treatment cost?


At the conclusion of this initial appointment, Dr. Judd will usually begin treatment.  This may include spinal adjustment, soft tissue massage, stretches and exercise.  He may also recommend massage through our Massage Therapy services.  Before leaving, Dr. Judd will share with you certain activities or positions to avoid, and at home exercises and/or stretches. He will also give any instructions for ice or heat application.
